Could anyone tell me the manufacturers of some pickup based Class C's. I find the van cab very uncomfortable.

Google Tiger motorhomes. I believe they are manufactured in the Columbia, SC area. It's a small C. Don't know is the cab over bed is going to be much bigger, Some truck campers have some decent size cabovers with full queen mattress in the north/south position.

The Tiger CX is available on any cab length & drivetrain Ford or Chevy F350/3500. I love mine!
Also Google for Xplorer (Ford or Dodge 350 /450, IIRC) & Host (Ford F450? F550?).
Have you tried a Chevy van? They've got a bit more room in the cab.

Check out Host. They make a lovely rig with a crew cab front end. Four luxurious seats with an aisle between to get into the back and a huge king size bed over the crew cab front.

In addition, look at chevy van cabs. They are more comfortable - though no longer produced, I think.

You might want to check out the Chevy chassis before ruling out the van-style RV. Understand the over-all length of the RV is relative to the chassis. For what it's worth, most people get used to the confined foot room in the Ford. Even my brother and his wife did, and that was a huge issue for them when shopping. Not an issue for them any longer with their Ford chassis today.
Another interesting point. The Ford van chassis front cab is 9" shorter than the Chevy van chassis. You get 9" more living space with the Ford versus Chevy. Or a 9" shorter motor home with the same size living quarters.

I am a big guy and looked for the same thing. I finally said I can live with theford and got a bornfree. It is ok but I wear long pants to drive as the doghouse get hot on a long trip. IT is doable but not as comfy as my Foretravel but then again it fits where many things will not and I do not have to tow a car behind me either.
