You're looking at a custom job, need to find a local place that can build one for you. See who makes custom truck racks for the trade guys (electrician, plumber, etc.) and see if they can help you out. My friend made a custom over cab rack for his Jet Ski, said he could have sold 20 a year from all the people that asked to buy one, but he wouldn't consider it due to liability. He always said if the person wanted to buy the materials, and come over he'd work with them to design and construct it, but he wouldn't just make it. When I asked for help with my ATV trailer, he said the same thing. I was the first one that ever took him up on the offer, we had a freakin' blast doing it.

I'm not sure how to link threads but if you search for 'toy hauler loading' there is a nice over the cab set up with a fifth wheel.

The bed mount racks are very popular here, they are called sled decks. They are primarily for hauling sleds in the winter but they are also used by us atv'ers. I have one on my truck so I can haul 2 quads and still tow the travel trailer. I'll have to post a pic of it sometime all loaded up, It's another way of doing the toyhauler but still having the truck to get the atv's to the trail head.

I have seen a 4-place that also went over the cab and was supported by struts down to the front bumper/frame but I'm pretty sure it was a custom job.

Custom job I bet. I've spoken to two guys that had setup's like that, one hauled two 90cc ATVs over cab without front supports, the other had supports and he carried two smaller sand quads. One guy told me he searched everywhere to buy a pre-made rack, couldn't find any and was told due to liability insurance and regulations.
