Anyone know what kind of Class C this is? It's got that slide out rear, and the extra axle for what? Very strange looking....almost European, but the photo was taken in Kentucky, and has the Ford chassis?

The "extra" axle is a TAG, used to prevent overloading the rear axle. Probably the rear slide is too much weight for stock chassis,

Looks like a passenger bus conversion.
It doesn't matter to me if you don't own a Mac.

I was also thinking bus conversion, or maybe with the bear on the side it was a zoo or safari tour bus?

The extra axle is a tag to carry extra weight, because the coachbuilder probably wanted to exceed the limits of the chassis he could get at the time. There were a number of C's and A gassers that got aftermarket tag axles in the mid to late '80s when RVs were growing faster than chassis were being up-rated.
More than one C manufacturer was using rear slideouts, before slideouts on the side became generally used. Winnebago had at least one rear-slide C, in a length that would be considered short today, but was pushing the chassis limits at the time. But the one in your picture is not the Winnebago.

It looks like a home-made mini-bus conversion. The 3rd axle looks to be an aftermarket tag, presumably because the weight of the slide/porch is too much for the rear of the bus.

even with the tag - look how it squats the rear when slide is out. i couldn't sleep like that...
