We will be Nova Scotia for 12 days. I have reservations in Cape Breton National Park. Should I reserve campgrounds for the other days?

Which 12 days?

There is a huge difference in numbers of people travelling / camping during July & August and โ€˜shoulder monthsโ€™.

Five islands for sure. Also Rissors and Graves Island Provincial Parks. Digby is a good spot as well.

If the question is "are there places we can dry camp?" then the answer was "yes" depending on where you happen to be. There's a Walmart in Halifax that we've used as an overnighter. There was one in Port Hawkesbury and I imagine there are others. If you're looking for rest areas to overnight, there may be some, but Canada doesn't seem to have as many as the US along our major highways. With that in mind, I wouldn't be afraid to try pot luck on the dry camping, but haven't been out there for 3 years, so things may have changed, like they did in Gaspe.
If you want guaranteed camping, reserve.
Mike. Comments are anecdotal or personal opinions, and worth what you paid for them.
